Spring Formal 2026 Information
Please review this document for information on the upcoming Koinonia Spring Formal. We are looking forward to having a wonderful evening.
Students will meet at ORCHARD VIEW CHURCH OF GOD on Thursday, April 23, 2026, at 4:00 pm. We will depart for our first stop by 4:45pm.
Cost is as follows:

Early Bird Tickets:      March 16 –23             $75 members/$85 non-members

Regular Tickets:         March 24 – April 10   $80 members/$90 non-members

Late Registration:      April 11 –18                $90 members/$100 non-members

All participants MUST submit a completed Parental Consent and Release Form along with
full payment NO LATER THAN SATURDAY, APRIL 18.


Guidelines
Spring Formal is open to ALL High School students. Students who will be 14 years of age by June 1st are also invited to attend.

Students will not be allowed to drive a vehicle at any time during the event. Please designate on the Consent Form how your student will be returning home after the event.
They may be (1) picked up by an adult by 12:15 AM at Orchard View Church of God, (2) get a ride home from an approved chaperone from Orchard View Church of God, 
(3) drive themselves (and siblings) home from Orchard View Church of God.  Any exception to these guidelines will require written permission from the parent.
Change in pick-up plans will not be allowed on the day of unless the parent contacts

Students are expected to stay at the event for the whole evening. If a student must leave, they MUST check out with Mrs. DeVries or Mrs. Ruse!!

In order for Koinonia students to participate, ALL Koinonia Family Account payments must be CURRENT as of March 30, 2026.

The Spring Formal is not meant to be a dating opportunity. Although Koinonia students may invite a friend to come for the evening, no physical displays of affection will be allowed
throughout the entire evening. This includes hand holding, kissing, going off to be alone anywhere, etc.
Opening doors for girls and treating girls like ladies is always acceptable.

Dress Code for the formal part of the evening:

Girls:
• All girls must be in gender appropriate clothing.
• All dresses must have at least one strap. (No strapless dresses)
• Girls must be sure that no cleavage is showing.
• Backs of dresses for girls may not be lower than the bottom of shoulder blades.
• Dress length, as well as slits in dresses, should not be higher than the knee.

Note: We realize that it is becoming difficult to find dresses that meet the dress code requirements.  We appreciate your efforts to find a dress that is modest and God honoring. 
Please use your judgment when selecting a dress, keeping in mind that this is a Christian Homeschool organization.

Boys:
• All boys must be in gender appropriate clothing.
• The appropriate dress includes trousers, a dress shirt with a collar, tie, dress shoes.
• No barn clothes, sweatshirts, t-shirts, blue jeans

Dress Code for 3 Mile Project – Students should bring a bag containing comfortable clothes and casual/athletic type shoes for use in playing different sports events & games.
Tennis shoes are required to participate in events such as the ninja course and the various courts (basketball, volleyball, pickle ball, etc).
Students will be able to change their clothes at 3 Mile Project.

Registration Closes on April 18, OR when the event is FULL.
You will need to COMPLETE, SIGN and SUBMIT:
1.) The Koinonia Spring Formal Parent Consent and Release Form (below)
2.) The 3 Mile Project Waiver

Everyone participating MUST have a signed CONSENT FORM from Koinonia, as well as a Waiver on file with 3 Mile Project, including guests of students.
Koinonia students are allowed to invite ONE non-Koinonia friend. If your student brings a friend to the Spring Formal, all the above requirements must be met,
including all signed forms and waivers.  If you would like to invite additional guests, you must get permission from Mrs. DeVries or Mrs. Ruse.

Consent Forms and Payments (cash or checks payable to Koinonia) can be turned in at West Cannon, sent US Mail to: Koinonia Home School Group c/o  Attn: Spring Formal,
or submitted online with credit card payment.

Parental Consent and Release Form

I give permission for my student to ride in a vehicle driven by any of the approved Koinonia chaperones from Orchard View Church of God to all locations indicated on this information sheet.

I understand and accept that Koinonia and its chaperones will not be held liable for accidents or injury that may occur during this event. I understand that in the event that medical treatment is required, every effort will be made to contact the listed parents/guardian. However, if I cannot be reached, I consent to and authorize the giving of all treatments, medications, and the performance of any other technical procedures, which are ordered by a medical technician and/or physician for the care of the above-named student. I also agree to accept all financial responsibility incurred as a result thereof and not hold responsible Koinonia or its chaperones.
I understand and agree that the above-named student will follow and adhere to the Koinonia Guidelines, as well as the directions and supervision of the chaperones and staff of Koinonia, and that failure to do so may result in disciplinary action, up to and including termination of their participation in this event. (Parents would then be required to come and pick up their student).
In consideration of my student’s participation in Koinonia’s Spring Formal event, THE UNDERSIGNED HEREBY FULLY WAIVES, RELEASES, DISCHARGES, AND AGREES TO INDEMNIFY AND HOLD HARMLESS (“Release”) Koinonia from any and all rights, claims, liabilities, and actions for losses, damages, or injuries to property or person that may arise out of or result from use of facilities, equipment, or properties by me or my child.
